Children entering seventh grade are required to receive one dose of Tdap (Tetanus, diphtheria and acellular Pertussis, adolescent/adult formulation) or Td (Tetanus and diphtheria, adult formulation) administered as a booster dose prior to entering the seventh grade.

The Madison County/London City Health District is holding a special clinic from 2 to 6 p.m. on Wednesday, Aug. 5 by appointment. The cost of the vaccine is $10.

Every child entering kindergarten in Madison County is required to have a physical examination and an up-to-date immunization record.

The Madison County/London City Health District offers physicals and immunizations for your convenience. The cost is $60 plus the cost of vaccines. Most insurance plans accepted. No one is turned away due to inability to pay.

Please remember, all physicals and immunizations must be completed and returned to the school on or before the first day of school.

Your child will also need a birth certificate to enroll in kindergarten. Birth certificates for every county in Ohio are available at the Madison County Health Department. You can come into the office to pick up the certificate from 8:30 a.m. to 4:15 p.m. Monday through Friday. We offer same-day service. The fee is $23.

If you cannot come in during our office hours, applications are available on the county website at Look for the Health Department link on the left side of the screen and then for the Office of Vital Statistics link. Mail it to the health department. We will mail the certificate to you the same day we receive your application.
