It’s that time of year again. Parents are registering their children for kindergarten. Every child entering kindergarten in Madison County is required to have a physical examination and an up-to-date immunization record.
It is not too early to have your child’s physical exam or immunizations done now. Please call the Madison County/London City Health District or your child’s physician today. Please remember, all physicals and immunizations must be completed and returned to the school on or before the first day of school. If you have any questions or you want to schedule an appointment, please call the health district at (740) 852-3065. No one is turned away due to inability to pay. We encourage parents not to wait too long as appointments do fill up fast.
Your child will also need a birth certificate to enroll in kindergarten. Birth certificates for every county in Ohio are available at the Madison County/London City Health District. You can come into the office to pick up the certificate from 8:30 a.m. to 4:15 p.m. Monday through Friday. We offer same-day service. The fee is $23. If you cannot come in during our office hours, applications are available on the county website at http://co.madison.oh.us. Look for the Health Department link on the left side of the screen and then for the Office of Vital Statistics link. Mail the application to the health district, along with the fee, and we will mail the certificate to you the same day we receive it.
